Lost of misplaced your card?

Please contact us if you have lost or misplaced your Carer Card. To get a replacement card, you will have to complete the Carer Card application form again, choosing card replacement as your option.

Renewing your card

Your Carer Card is valid for 12 months. An expiry date is printed on the front of the card. If the expiry date has passed, your card will no longer be valid and will need to be renewed. To renew your card, you will have to complete the Carer Card application form again, choosing card renewal as your option.
