Provider Terms and Conditions

About the Carer Card

The Carer Card is provided to unpaid carers who are known to Lanarkshire Carers Centre Ltd and who provide substantial care to the person(s) they care for.

The Carer Card remains the property of Lanarkshire Carers Centre Ltd.

The Carer Card is valid for a period of 12 months; at which time it can be renewed by the carer.

Carers who no longer provide unpaid care due to a change in their circumstances are entitled to continue using the Carer Card for a period of 3 months after notifying Lanarkshire Carers Centre Ltd of any such changes.

All Carer Cards are issued with photographic identification, a start date, an expiry date and a card issue number.

We reserve the right to change the appearance and conditions of the Carer Card at any time. If we do so, we will inform you of any such changes.

Misuse of Card

You have the right to refuse or withhold any Carer Card if you suspect it is being used inappropriately or fraudulently. Where you have withheld a card, you must notify us immediately and the reason(s) for doing so.

You reserve the right not to provide goods and services offered under the terms and conditions of this agreement if no card is presented or if a card is invalid.

Supplying Goods and Services

You agree to provide the goods and services as detailed within the agreement made with us to any person(s) in receipt of a valid Carer Card issued by Lanarkshire Carers Centre Ltd.

In providing these goods and services, you agree to do so in a way that does not constitute inappropriate, offensive, discriminatory, illegal, fraudulent, harmful or unlawful behaviour or practice. We reserve the right to report any such wrongdoing to relevant governing bodies and/or authorities.

We may offer similar goods and services through other providers, including your competitors. It is important that you are aware of this before agreeing to the terms and conditions. We would not normally refuse other similar companies providing an offer or discount to Carer Card holders unless this is explicitly specified as a condition in your agreement with us. However, we exercise the right to refuse any such condition prior to this agreement being signed.

Promotion of the Carer Card Scheme

By signing this agreement, you give us consent to promote your business or organisation as a provider associated with the Carer Card Scheme through our website, social media or any other relevant publications we produce.

We agree to promote the goods and services you have stipulated in this agreement as a provider associated with the Carer  Card Scheme.

You may also promote any goods and services as a provider associated with the Carer Card Scheme, which must be reviewed and approved by us before publication. In doing so, you should associate the Carer Card Scheme with Lanarkshire Carers Centre Ltd using our logo including any associated branding or guidelines stipulated by us.

You must notify us immediately of any changes to the goods and services you are providing through the Carer Card scheme.

Intellectual Property

You must own the copyright for any logos, branding or imaging you provide to us for use in publicity material. If we decide to use this in our publicity material, you must give us your explicit consent to do so.

If we suspect that any logos, branding or imaging you have provided to us infringes copyright law, we reserve the right not to use it in publicity material.

Any logos, branding or imaging associated with Lanarkshire Carers Centre Ltd is copyrighted and should not be used without our explicit consent.

Use of Personal Information

We would encourage you not to capture any personal information regarding the Carer Card holder unless it is necessary to do so for the type of goods and services you are providing. If you do capture any personal information about the Carer Card holder, you must do so with their explicit consent and process any such information in line with current Data Protection legislation.

Lanarkshire Carers Centre Ltd cannot and will not provide any Personal Information regarding Carer Card holders without their explicit consent to do so. Any such request must be made to us in writing.

Changes to these terms and conditions

We reserve the right to change the terms and conditions of this agreement at any time. If we do so we will notify you in writing at least 30 days before making any such changes, providing details of the changes made.

You agree to notify us as soon as possible if you are no longer able to honour any of the terms and conditions made under this agreement.

Renewing the agreement

We will contact you at least 30 days prior to the end date of the agreement to determine if you wish to renew your agreement. In doing so you will be given the opportunity to review and amend the terms and conditions of your agreement with us. A new agreement will be produced.

Lanarkshire Carers Centre Ltd reserves the right to refuse or terminate a renewal agreement under the conditions set out under the ‘Ending this agreement’ section.

Ending the agreement

You can terminate the agreement by giving us 30 days’ notice prior to the end date you have agreed with us.

You may also terminate the agreement at any time under the following conditions:

  • If we change the terms and conditions of this agreement in a way that is no longer suitable or appropriate to you. You must stipulate the reason(s) for this.
  • You are no longer able to honour the agreement. You must stipulate the reason(s) for this.
  • We fail to meet or uphold any of our obligations to you.
  • If you cease operations or transfer undertakings

Lanarkshire Carers Centre Ltd reserves the right to terminate or amend the terms and conditions set out within the agreement at any time. We will provide notice to the named contact(s) above and our reason(s) for doing so. This includes, but is not limited to:

  • If you refuse to provide goods or services to Carer Card Holders.
  • If you amend the terms and conditions of this agreement without our consent.
  • If the goods or services you are providing are deemed inappropriate, offensive, discriminatory, illegal, fraudulent, harmful or unlawful. We reserve the right to report any wrongdoing to the relevant governing bodies and/or authorities.
  • If the goods or services provided are no longer relevant or deemed unsuitable.
  • If the Carer Card Scheme comes to an end.
  • If we cease operations or transfer undertakings.
